In accordance with Porter/Rosasco's letter(Serial No. KJ-IC03-032) dated August 4, 1992, deduct the
cost of HLM's design review of the out-of-tolerance cast-in-place columns for eccentric loading from
the precast concrete structure.
Contractor agrees to accept this LUMP SUM CHANGE ORDER as FINAL SETTLEMENT for this extra work and all related impacts
due to this contract change for the LUMP SUM AMOUNT of $ -500.00 and 0 days.

A. GENERAL INFORMATION
AlA Document G604 is J new document. It is intended to iormallZe the procedures for authorizing supplemental
professional Jctions such as expJnding the scope of Bdsic Services, Incurring RelnlbursJhle Expenses or proceed-
ing with certain Additional Services. It should only be used to record actions authorized in accordance with an
earlier Agreement for professional services. It will provide J written record oi such authorizations, giving particu-
lars of Jctivlties, time spJns and compensation involved.
B. COMPLETING THE G604 FORM
The dJte to be idled in on the iirst line should be the date of the original Agreement between the parties.
Similarly, "The Project" should he described JS it is on the Agreement form,
The document is flexible for use by any two parties. Thereiore, the Owner could authorize the Architect to take
some actIOn or the document could be used ior Architect-Engineer, ;'\rchitect,lnterior Designer, or other relation-
ships. Fill in the blanks appropriately.
One or more actions can be designated and each should be fully described after the "as follows:".
Complete proviSions for compensation dnd time of performance should be listed. Often the basis of compensa-
tion will be covered in the Agreement, such as for Additional Services. vVhether by reference, restatement or new
Jgreement, full particulars of compensation and time should be Included here. For example: "Compensation for
.Additional Services shall be in accordance with Paragraph 14.4 of the Agreement."; "The time for performance of
the Design Development services shall be extended by twenty-one (21) calendar days."
Ii additIOnal sheets are used to describe authorized action, compensation or time of performance, they should be
identiiied at the applicdble plJce, and the number of additional sheets should be stclted. It is advisable for both
parties to initiJI and date all such sheets.
The document should be signed in a manner consistent with that of the original f\greement. In case of any ques-
tion, it is appropriate to ascertain whether the other party has authority to authorize or agree to perform the activ-
ities contemplated,
