You are directed to make the following changes in this contract; all other provisions of the contract shall, except as
specifically changed herein, remain in full force and effect.
In accordance with the Contractor's Post-Bid Substitution Request dated October 7, 1994, recommended
as noted by the A/E, incorporate the following revisions to the building electrical design;
- Revise the primary transformer secondary voltage from 120/208 to 277/480 volts. Conduits and
wiring for secondary voltage to be as indicated on revised drawings issued by A/E.
- Revise the NEMA 3R, 2,000 A main switch at the primary transformer to a NEMA 1, t.OOO A main~
switch located in electrical room #1108. Feeder conduit and wiring to be relocated from~l~C/2 ~
to C/1 and rerouted to electrical room #1108 via RGC under the 1st floor slab. ~ ~
- Relocate the Emergency Generator ATS from the penthouse to electrical room #1108 ~~ise ~
voltage as required. Conduits and wiring to be as indicated on revised drawings issued ~,~e.~/E. ~
- Relocate the UPS from the penthouse to electrical room #1138 or #1140 as dircted by~ne A/E.
- Revise the Emergency Generator, elevators, and mechanical equipment as indicatedtn1ne ~
substitution request to 480 volt. Contractor to coordinate with subcontractors and suppl~fiill
equipment submittal corrections required for voltage change. Feeders and disconnects for tliis '. -0
equipment to be revised by Contractor in accordance with NEC and Building Code standards.~<. -. ~
- Perform panel revisions as outlined to incorporate 480 volt changes and use of th~,oPS. built ~
electrical panel. Add step down transformers as outlined except that use of the existing'County ~
owned transformer shall not be available until the actual Substantial Completion Date of iRe
project. All sub-feeders, branch circuits, and panel revisions and layouts performed by Contractor
shall be in accordance NEC and Building Code standards.
Contractor agrees to accept this LUMP SUM CHANGE ORDER as FINAL SETTLEMENT for this extra work and all related impacts
due to this contract change for the LUMP SUM AMOUNT of $ -15,000.00 and 0 days.

, A. GENERAL INFORMATION
AlA Document G604 is a new document. It is intended to formalize the procedures for authorizing supplemental
profe,ssionalactions such as expanding tbe scope of Basic Services; incurring Reimbursable Expenses or proceed-
ing with certain Additional SerYices. It sl10uld only be used to record actions authorized in accordance with an
earlier Agreement for professional services. It will provide a written record of such authorizations, giving particu-
lars of activities, time spans and compen~ation involved.
B. COMPLETING THE G604 FORM
The date to be filled in on the first line should be the date of the original Agreement between the parties,
Similarly, ,iThe Project" should be described as it is on the Agreement form.
The document is flexible for use by any two parties.. Therefore, the Owner could authorize the Architect to take
some action or the document could be used for Architect-Engineer, Architect-Interior Designer, or other relation-
ships. Fill in the blanks appropriately.
One or more actions can be designated and each should be fully described after the "as follows:".
Complete provisions for compensation and time of performance should be listed, Often the basis of compensa-
tion will be covered in the Agreement, such as for Additional Services, Whether by reference, restatement or new
agreement, full particulars of compensation and time should be included here. For example: "Compensation for
Additional Services shall be in accordance with Paragraph 14.4 of the Agreement."; "The time for performance of
the Design qevelopment services shall be extended by twenty-one (21) calendar days."
If additional sheets are used to describe authorized action, compensation or time of performance, they should be
identified at the applicable place, and the number of additional sheets should be stated, It is advisable for both
parties to initial and date all such sheets. ~ c
The dcrcument should be signecHn a manner consistent with that of the original Agreement.-In case of any ques-
tion, it is appropriate to ascertain whether the other party has authority to authorize or agree to perform the activ-
ities contemplated.
