Hancafkl}v Deputy Clerk CY At the February 15, 2006, Board of County Commissioner's meeting the Board granted approval and authorized execution of the following: I Amendment 001 to the Home Care for the Elderly (HCE) Contract #KH472 between the Alliance for Aging, Inc. and the Monroe County Board of County Commissioners (Monroe County Social Services/In-Home Services Program) for Fiscal Year July 1, 2004 through June 30, 2005, to decrease by $1,591.00 the HCE contract allocation for subsidies and increase the HCE Contract #KH472 allocation for Case Management by $1,591.00. The total contract amount has not changed. Amendment 001 to the Home Care for the Elderly (HCE) Contract #KH572 between the Alliance for Aging, Inc. and the Monroe County Board of County Commissioners (Monroe County Social Services/In-Home Services Program) for Fiscal Year July 1, 2005 through June 30, 2006, to decrease by $2,01.00 the HCE contract allocation for subsidies and increase the HCE Contract #KH572 allocation for Case Management by $2,010.00. The total contract amount has not changed. Enclosed are four duplicate originals of each of the above-mentioned, executed on behalf of Monroe County, for your handling. Please be sure to return the fully executed "Monroe County Clerk's Office Originals" and the "Monroe County Finance Department's Originals" as soon as possible. The purpose of this amendment is to decrease by $1,591.00 the DCE contract allocation for subsidies and increase the DCE contract allocation for Case Management by $1,591.00. The total contract amount for HCE doesn't change. 2. This amendment shall begin on Ju Iy 1, 2004 or on the date it has bee n signed by both parties, whichever is earlier. All provisions in the contract and any attachm ents thereto in conflict with this amendment shall be and are hereby changed to conform with this amendment. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract are hereby amended to conform with this amendment. This amendment and all its attachments are hereby made a part of the contract.
