SECTION THREE
CONTRACT
THIS AGREEMENT, made and entered into this lAth day of November,
1996, A.D., by and between MONROE COUNTY, FLORIDA, party of the
first part (hereinafter sometimes called the "COUNTY"), and Airmax
Service Corporation, party of the second part (hereinafter
sometimes called the "CONTRACTOR").
WITNESSED: That the parties hereto, for the consideration
hereinafter set forth, mutually agree as follows:
3.01
THE CONTRACT DOCUMENTS
A. The Contract Documents consist of this Contract Agreement,
Instructions to Bidders, Contract Specifications, Non-
Collusion Affidavit, Ethics Clause, Drug-Free Workplace Form,
the Insurance Documents (INSCKLST 1-5, GL1, VL1, WC1, GIRl,
GIR2), Indemni f ication and Hold Harmless (TCS). These form
the Contract, and all are as fully a part of the Contract as
if attached to this Agreement or repeated herein.
In cases of conflict within the described Contract
in Article 3.0 1A of this Contract Agreement, the
precedence shall be as follows:

SCOPE OF THE WORK
The CONTRACTOR shall provide air conveyance system cleaning,
including all necessary supplies and equipment required in the
performance of same, and perform all of the work described in the
Specification Package entitled:
CON-I
Contract Specifications
Air Conveyance System Cleaning
Monroe County Courthouse Annex
500 Whitehead Street
Key West, Monroe County, Florida
and his bid dated October 6, 1996, each attached hereto and
incorporated as part of this contract document. The specifica-
tions shall serve as minimum contract standards, and shall be the
basis of inspection and acceptance of all the work.
3.03
PERMITS
The Contractor shall obtain all required permits applicable for
central alc environmental cleaning from the City of Key West
Building Department prior to commencement of work, permit to be
displayed at the project site during the period of work.
Contractor shall be responsible for compliance with all
procedures, scopes of work, and requirements noted in the attached
general specifications prepared by EE&G Science and Management,
attached hereto and incorporated as part of this contract
document.
3.04
CONTRACTOR LICENSES
The Contractor shall be a licensed contractor and shall provide
proof thereof by attachment of all current licenses upon execution
of this agreement.
3.05
PAYMENT
In accordance with the provisions fully set forth in the Request
for Bids and subject to additions and deductions as provided, the
Owner shall pay to the Contractor as follows:
The Contract sum shall be paid in full upon final acceptance by
the County. Total contract amount shall not exceed Twenty-Five
Thousand Four Hundred Dollars and No Cents ($25,400.00).

3.06 CONTRACTOR'S ACCEPTANCE OF CONDITIONS
A. Any ambiguity or uncertainty in the Specifications shall be
interpreted and construed by John King, Building Maintenance
Manager, Monroe County Public Facilities Maintenance, and his
decision shall be final and binding upon all parties.
B. The passing, approval, and/or acceptance of any part of the
work or material by the COUNTY shall not operate as a waiver
by the COUNTY of strict compliance with the terms of this
Contract, and Specifications covering said work. Failure on
the part of the CONTRACTOR, immediately after Notice to Cor-
rect materials and/or delivery shall entitle the COUNTY, if it
sees fit, to correct the same and recover the cost of
replacement from the CONTRACTOR, who shall in any event be
liable to the COUNTY for all damage, loss, and expense caused
to the COUNTY by reason of the Contractor's breach of this
Contract and/or his failure to comply strictly and in all
things with this Contract and with the Specifications.

3.08
INDEPENDENT CONTRACTOR
At all times and for all purposes under this agreement the CON-
TRACTOR is an independent contractor and not an employee of the
Board of County Commissioners for Monroe County. No statement
contained in this agreement shall be construed so as to find the
contractor or any of his/her employees, contractors, servants, or
agents to be employees of the Board of County Commissioners for
Monroe County.
3.09
ASSURANCE AGAINST DISCRIMINATION
The CONTRACTOR shall not discriminate against any person on
basis of race, creed, color, national origin, sex, age, or
other characteristic or aspect which is not job related, in
recruiting, hiring, promoting, terminating, or any other

affecting employment under this agreement or with the provision of
services or goods under this agreement.
3.10
ASSIGNMENT/SUBCONTRACT
The CONTRACTOR shall not assign or subcontract any of its respon-
sibilities under this agreement, except in writing and with the
prior written approval of the Board of County Commissioners for
Monroe County, which approval shall be subject to such conditions
and provisions as the COUNTY may deem necessary. This agreement
shall be incorporated by reference into any assignment or subcon-
tract and any assignee or subcontractor shall comply with all of
the provisions of this agreement. Unless expressly provided for
therein, such approval shall in no manner or event be deemed to
impose any obligation upon the COUNTY in addition to the total
agreed-upon price of the services/goods of the contractor.
3.11
COMPLIANCE WITH LAW
In providing all services/goods pursuant to this agreement, the
contractor shall abide by all statutes, ordinances, rules and
regulations pertaining to, or regulating the provisions of, such
services, including those now in effect and hereinafter adopted.
Any violation of said statutes, ordinances, rules and regulations
shall constitute a material breach of this agreement and shall
enti tIe the COUNTY to terminate this contract immediately upon
delivery of written notice of termination to the contractor.
3.12
INSURANCE
Upon execution of this agreement, the CONTRACTOR shall furnish the
COUNTY Certificates of Insurance indicating the minimum coverage
limitations as indicated by an "X" on the attached forms
identified as INSCKLST 1-5, as further detailed on forms WC1, GL1,
VL1, GIRl & GIR2, each attached hereto and incorporated as part of
this contract document.
3.13
FUNDING AVAILABILITY
In the event that funds from the Public Works Division Contractual
Services are partially reduced by the Board of County Commis-
sioners or cannot be obtained or cannot be continued at level
sufficient to allow for the purchase of the services/goods speci-
fied herein, this agreement may then be terminated immediately at

the option of the Board of County Commissioners by written notice
of termination delivered in person or by mail to the contractor.
The Board of County Commissioners shall not be obligated to pay
for any services provided by the contractor after the contractor
has received written notice of termination.
3.14
PROFESSIONAL RESPONSIBILITY
The CONTRACTOR warrants that it is authorized by law to engage in
the performance of the activities encompassed by the project
herein described, subject to the terms and conditions set forth in
the Contract Specifications entitled "Contract Specifications /Air
Conveyance System Cleaning/Monroe County Courthouse Annex", which
is attached hereto and incorporated herein as a part of this
contract/agreement. The provider shall at all times exercise
independent, professional judgment and shall assume professional
responsibility for the services to be provided. Continued funding
by the COUNTY is contingent upon retention of appropriate local,
state, and/or federal certification and/or licensure of
contractor.
3.15
NOTICE REQUIREMENT
Any notice required or permitted under this agreement shall be in
writing and hand delivered or mailed, postage prepaid, to the
other party by certified mail, returned receipt requested, to the
following:

3.16
GOVERNING LAWS
This Agreement is governed by the laws of the State of Florida.
Venue for any litigation arising must be in Monroe County, Flori-
da.
3.17
COMMENCEMENT AND COMPLETION
This is a sixty (60) hour contract. The actual start date of the
project will be determined by the Facilities Maintenance Director.

All work must be performed at 5:00 p.m. Friday, all work must be
completed by 5:00 a.m. the following Monday. All times are
Eastern Standard Time.
IN WITNESS WHEREOF the parties hereto have executed this Agreement
on the day and date first above written in four (4) counterparts,
each of which shall, without proof or accounting for the other
counterparts, be deemed an original Contract.

LAST FIVE AIR CONVEYANCE SYSTEM CLEANING PROJECTS:
1. Greenwich Air Services, Miami International Airport. Contact
Peter Beck, 870-8048. Project involved cleaning, sanitizing and
coating with Portersept two air conveyance systems serving offices
and library; in addition two air conveyance systems serving
plasma work areas and required removal of heavy metals from
ductwork and refurbishment of air handling units with IMCOA
insulation. The total contract amount was $36,795.00. Work
completed October 5, 1996.
2. School Board of Brevard County: Oak Park Elementary School;
Total purchase Order amount: $57,860.00. Work completed
October 4, 1996. Clean and refurbish 33 air handling units and
ductwork; clean and refurbish 41 unit ventilators. Contact
Richard Smith or Jim Varney in Facilities: 407-633-3496.
3. School Board of Brevard County: Discovery Elementary; Total
Purchase order amount: $4,914.00. Clean, refurbish with
IMOCA insulation, coat ductwork with Porter sept and service
condensing units in 14 Bard air conditioning units attached to
classroom trailers. Completion September 30, 1996. Contact
Richard Smith, Jim Varney in facilities.
4. Seminole Community College; Total Purchase order amount:
$104,400.00. Clean, sanitize, refurbish seven air handling
systems and ductwork serving Building B. HV AC system
consisted of High pressure ductwork to 54 fan powered VAV
units and low pressure ductwork. Air handling units were
refurbished with IMOCA insulation; VAV boxes were cleaned
sanitized and coated with antimicrobial coating. Contact Mr. Roy

5. Monroe County School Board: Plantation Key Elementary School

AIRMAX SERVICE CORPORATION DRUG-FREE WORK PLACE POLICY
In a commitment to safeguard the health of our employees and to provide a safe working
environment for everyone, we have established a Drug-Free Work Place Program for Airmax
Service Corporation. This policy is implemented pursuant to the Drug-Free Work Place
Program requirements under F.S. 440-102 and the rules of the Department of Labor and
Employment Security, Division of Worker's Compensation.
The essential parts of this policy are:
1. The company prohibits the illegal use, possession, sale, manufacture or distribution of
drugs, alcohol or other controlled substances on its property/jobs. It is also against
company policy to report to work or to work under the influence of drugs or alcohol.
2. Drug testing of applicants:
a. All applicants considered final candidates for a position will be tested for the
presence of drugs as part of the application process.
b. Applicants will be asked to sign the Consent to Pre-employment form. If an
applicant refuses, he or she will not be considered for employment and the
employment application process will be terminated.
c. If an applicant's test is confirmed positive, the applicant will not be considered
for employment at that time and will be informed that he or she has failed to
meet employment standards.
3. Testing of employees:
a. Reasonable Suspicion Testing - Employees will be tested when there is a
reasonable suspicion that an employee is using or has used drugs.
b. Routine Fitness for Duty Testing - Employees will be drug tested if the test is
conducted as part of a routinely scheduled fitness-for-duty medical examination.
c. Follow-up Testing - All employees who have been determined to have used
drugs or alcohol will be subject to unannounced follow-up tests.
d. Additional Testing - Additional testing may also be conducted as required by
applicable state or federal laws, rules or regulations as deemed necessary by the
company.
4. Disciplinary action:
a. In the case of a first-time violation of the company's policy, including a positive
drug or alcohol result (without evidence of use, sale, possession, distribution,
dispensation or purchase of drugs or alcohol on company property or while on
duty), the employee will be subject to discipline up to and including discharge.
b. The company may suspend employees without pay under this policy pending
the results of a drug test or investigation.
c. Any employee using, selling, purchasing, possessing, distributing or dispensing
drugs or alcohol on duty or on company property will be discharged.
5. All information, interviews, reports, statement memoranda and drug test results, written
or otherwise, received by the company as part of this drug testing program are
confidential communications. Unless authorized by state laws, rules or regulations,
the company will not release such information without a written consent form signed
voluntarily by the person tested.
6. Attachment B is a Drug Use Information form which is a confidential report which must
be filled out by job applicants and employees before and after being drug tested. This
form permits individuals to list all prescription and non-prescription drugs they are
currently using or have relevant to the test.
7. Attachment C is a list of the most common medications by brand name or common
name and chemical name which may alter or affect a drug test.
8. Any applicant who refused to submit to the pre-employment drug test will be ineligible
for hire.
9. Any employee who refuses to submit to a drug test may be terminated from
employment or otherwise disciplined by the company. An injured employee who
refuses to submit to a drug test, in addition to the above, forfeits his eligibility for .all
worker's compensation medical and indemnity benefits.
10. Attachment D is a list of names, addresses and telephone numbers of employee
assistance programs and local alcohol and drug rehabilitation programs available to
employees.
11. A job applicant or employee who receives a positive confirmed drug test result may
contest or explain the result to the employer within 5 days after written notification of
the positive test result. If a job applicant's or an employee's explanation or challenge
is unsatisfactory to the employer, the person may contest the test results.
12. A job applicant or employee has the responsibility of notifying the drug testing
laboratory of any administrative or civil action bought pursuant to Chapter 440, Florida
Statutes. The lab will maintain the sample until the case or administrative appeal is
settled.
13. The following is a list of all drugs, (described by brand name, common name and/or
chemical name) for which the employer may test:

14. Job applicants and employees have the right to consult the testing laboratory for
technical information regarding prescription and [Ion-prescription medication.
15. To ensure that drugs and alcohol do not enter or affect the work place, the company
reserves the right to search all vehicles, container, lockers or other items on company
property in furtherance of this policy. Individuals may be requested to display personal
property for visual inspection upon company request.
16. Failure to consent to search or display for visual inspection will be grounds for
termination or reason for denial of access to company premises.
17. Searches of employee's personal property will take place only in the employee's
presence. All searched under this policy will occur with the utmost discretion and
consideration for the employee involved.
18. Details of this policy may be obtained from Airmax Service Corporation.
19. The contents of these drug and alcohol guidelines are presented as statements of the
company's current policy and may be changed and updated by the company. These
guidelines are not intended to create a contract between the company and employee.
Nothing in these guidelines binds the company to a specific or definite period of
employment or to any specific policies, procedures, actions, rules or terms and
conditions of employment.
20. Employees, as a condition of employment are required to abide by these guidelines.

INTERPRETATIONS, CLARIFICATIONS, AND ADDENDA
No oral interpretations will be made to any bidder as to
the meaning of the contract documents. Any inquiry or
request for interpretation received seven (7) or more
days prior to the date fixed for opening of bids will be
given consideration. All such changes or interpreta-
tions will be made in writing in the form of an addendum
and, if issued, will be mailed or sent by available
means to all known prospective bidders prior to the
established bid opening date. Each bidder shall
acknowledge receipt of such addenda in the space
provided therefore in the bid form. In case any bidder
fails to acknowledge receipt of such addenda or
addendum, his bid will nevertheless be construed as
though it had been received and acknowledged and the
submission of his bid will constitute acknowledgment of
the receipt of same. All addenda are a part of the
contract documents and each bidder will be bound by such
addenda, whether or not received by him. It is the
responsibility of each bidder to verify that he has

GOVERNING LAWS AND REGULATIONS

The bidder is required to be familiar with and shall be
responsible for complying with all federal, state and
local laws, ordinances, rules, and regulations that in
any manner affect the work.

The bidder shall include in his bid prices all sales,
consumer, use, and other taxes required to be paid in
accordance with the law of the State of Florida and the

PREPARATION OF BIDS

his authority to sign the bid must be submitted. The
bidder shall state in the bid the name and address of
each person interested therein.

Basis for Bidding: The price bid for each item shall be
on a unit price basis according to the form of the bid.
The bid prices shall remain unchanged for the duration of
the contract and no claims for cost escalation during the
progress of the work will be considered.
SUBMISSION OF BIDS
Two (2) signed originals and one (l) copy of each bid
shall be submitted in a sealed envelope which shall be
marked so as to clearly indicate its contents and the
name of the bidder. If forwarded by mail, the above
mentioned envelope shall be enclosed in another envelope
addressed to the entity and address stated in the Notice
of Calling For Bids, and preferably by special delivery,
registered mail; if forwarded otherwise than by mail, it
shall be delivered to the same address. Bids will be
received until the date and hour stated in the Notice of
Calling For Bids.
WITHDRAWAL OF BID
Any bid may be withdrawn prior to the time scheduled in
the Notice of Calling For Bids for the opening thereof.
A bid may also be withdrawn thirty (30) days after the
date of the opening of the bids, provided that the bidder
has not been notified that his bid has been accepted.
MODIFICATION OF BIDS
Written bid modification will be accepted from bidders if
addressed to the entity and address indicated in the
Notice of Calling For Bids and received prior to bid due
date and time.

A bidder may modify his bid by telegraphic communication
at any time prior to the scheduled closing time for
receipt of bids, provided such telegraphic communication
is received prior to the closing time, and provided
further, the COUNTY is satisfied that a written confirma-
tion of the telegraphic modification over the signature
of the bidder was mailed prior to the closing time. The
telegraphic communication should not reveal the bid
price, but should provide the addition or subtraction or
other modification so that the final prices or terms will
not be known until the sealed bid is opened. If written
confirmation is not received within two days from the
closing time, no consideration will be given to the
telegraphic modification.
RECEIPT AND OPENING OF BIDS
Bids will be received until the designated time and will
be publicly opened and read aloud at the appointed time
and place stated in the Notice of Calling For Bids. The
person whose duty it is to open them will decide when the
specified time has arrived and no bids received
thereafter will be considered. No responsibility will be
attached to anyone for the premature opening of a bid not
properly addressed and identified. Bidders or their
authorized agents are invited to be present.
DETERMINATION OF SUCCESSFUL BIDDER
Until the final award of the contract, the COUNTY does
not bind to accept the minimum bid, but reserves the
right to reject any and all bids and to waive technical
errors and irregularities as may be deemed best for the
interests of the COUNTY. Bids which contain modifica-
tions, are incomplete, unbalanced, conditional, obscure,
or which contain additions not requested or irregulari-
ties of any kind, or which do not comply in every respect
with the Instructions to Bidders, and the contract
documents, may be rejected at the option of the COUNTY.
AWARD OF CONTRACT

The COUNTY reserves the right to reject any or all bids,
or any part of any bid, to waive any informality in any
bid, or to re-advertise for all or part of the work

The contract will be awarded to the qualified bidder
complying with the applicable conditions of the contract

The COUNTY also reserves the right to reject the bid of a
bidder who has previously failed to perform properly or
to complete contracts of a similar nature on time.
EXECUTION OF CONTRACT
The bidder to whom a contract is awarded will be required
to return to the COUNTY four (4) executed counterparts of
the prescribed contract together with the required
certificate of insurance within fifteen (15) days from
the date of notice of acceptance of the bidder's bid.

SCOPE OF WORK
The following constitutes a scope of work only. The items listed are specific to the described air
conveyance system cleaning (ACS) job only. In addition to the items in the scope of work, the
contractor is expected to abide by all relative guidelines included in The Specifications and all
applicable local, state, and federal regulations.
VENTILA TION SYSTEM MATERIALS
The material to be cleaned includes the following:
1st floor-
Interior of main air handler unit only
3rd floor-
Interior of two air handler units and all associated ductwork
4th floor-
Interior of two air handlers and all associated ductwork
All quantities are to be determined by the contractor through field inspection and/or review of
mechanical plans, if available.
LOCA nON OF MATERIAL
The areas to be cleaned are on the first, third, and fourth floors of the Monroe County
Courthouse Annex building located at 500 Whitehead Street in Key West, Florida.
PROJECT DATES AND DURATION
The actual start date of the project will be announced at a later date. All work must
performed on a single weekend. The building will be made available to complete the work
starting at 5:00 PM Friday, all work must be completed by 5:00 AM the following
Monday. All times are in Eastern Standard Time.
INITIAL PREPARATORY WORK
Access to the work areas shall be restricted to prevent unauthorized personnel from
entering the building. Barrier tape and/or warning signs shall be placed at all entrances
to the work area.
Lockout all existing power to the ACS by switching off all breakers servicing the system.
Label breakers with tag indicating "DANGER - circuit being worked on". Sign and date

danger tag. Lock panel and have all keys under the control of the contractor's
superintendent or Consultant. This work shall be performed by the Owner.
Each room throughout the work area shall be suitably protected from falling debris with
a polyethylene drop cloth (10' x 10' minimum size) prior to the initiation of cleaning
operations. Any ventilation ductwork to be entered is to be suitably supported and braced.
All registers, grilles and diffusers in the affected duct system will be fitted with critical
barriers to intercept airborne dust and contaminants, except those being accessed during
actual cleaning operations.
CLEANING METHODS
All grilles, diffusers, fan blades, and associated parts of the fan assembly which are non
electrical (e.g. housing) shall be removed and immersed in a 10 % bleach-water solution
for a minimum of five minutes. After soaking in the bleach solution, all equipment shall
be further cleaned using a brush or other suitable method until it is free of all visible
debris. All equipment should be thoroughly dried prior to being reinstalled.
All interior portions of the HV AC system including the air handlers and ductwork shall
be initially treated with an approved biocide1. The biocide must be allowed at least five
minutes of contact time prior to commencing with further cleaning work, and shall be used
in accordance with the manufacturer's recommendations. All insulation lining inside the
air handler units and ductwork shall be removed and disposed of.
Cleaning of the air handlers and ductwork shall be performed using mechanical brushes,
scrapers, HEP A vacuums, hand-held brushes, and sponges. Alternate or additional
methods of cleaning proposed by the contractor will be considered if they perform equally
or better than those listed above. Any requests for use of alternate methods should be
included with the bid package. Pressure washers or compressed air will not be permitted
for cleaning purposes. No cleaning methods shall be used which could potentially damage
the components of the ACS or alter the integrity of the system.
Cleaning of the coils shall not cause any appreciable damage to, displacement of, or
erosion of the coil surface, and shall conform to coil manufacturer recommendations, if
available. Any corrosion or residue buildup in the drain pans shall be cleaned to the
greatest extent possible.

RESPIRA TORY IPERSONNEL PROTECTION
Half-face negative pressure respirators with High Efficiency Particulate Air (HEPA) filters
shall be the minimum protective equipment required for all cleaning work. Disposable
protective clothing is encouraged to be used by the contractor for the purpose of
minimizing dirt and debris from being tracked throughout the building.
UTILITIES
Water and electricity are available from the facility. The contractor is responsible for
supplying sufficient power for the operation of all equipment used by both the Contractor
and the Consultant. Alteration of existing electrical systems shall be conducted by a
licensed electrician. All electrical cords and equipment entering the work area shall include
ground fault interrupters (GFI) in line with the supplied current prior to the current
entering the work area. Contractor shall supply sufficient lighting for all phases of material
removal.
FINAL INSPECTIONS
A final inspection of the HV AC system shall be performed by the Consultant upon notice
from the contractor that all cleaning work has been completed. The inspection will be
performed using borescopic methods. All surfaces within the ACS must be clean and free
of visible debris for the inspection to be satisfactory. Any areas found to be exhibit visible
debris buildup shall be recleaned at the contractor's expense until it is acceptable to the
Consultant.
APPLICA TJON OF SEALANT
After successful completion of the final inspection, an approved I anti-microbial sealant
shall be applied to all interior surfaces within the ACS except where it will interfere with
the proper operation of any mechanical equipment or alter the fire rating of any
equipment.
SPECIAL CONDITIONS
1. The contractor shall submit a list of all chemicals, including MSDS information
proposed for use on the project with their bid package.
2. Contractor, Owner, and Consultant shall inspect for any pre-existing damage to the
facility prior to beginning work. Contractor shall be responsible for repairing any
damage that is determined by Owner and Consultant to have been caused during

material removal operations. Any and all repairs must be conducted within the
allotted project time frame. If repairs are not completed by the project completion
date and time, the Contractor may incur liquidated damages as indicated in contract
documents. Each work area shall be left as clean or cleaner than before the work
began as determined by the Consultant.
3. All insulation lining the interior portions of the air handler units shall be
completely removed, disposed of, and replaced within the specified time frame.
All insulation shall be replaced with a fire retardant flexible unicellular material.
4. The ductwork on floors three and four have extractors located inside which may
obstruct any cleaning devices from passing completely through.
5. The contractor is also required to clean the tops of the ceiling tiles and all other
accessible/reachable objects in the ceiling space at each return air grille on the third
and fourth floors. The extent of the area to be cleaned shall be limited to that area
which are reachable upon removal of the return air grilles.
6. The filter tray on the 1st floor AHU needs repair so that the filter fits properly.
In addition, a P-trap needs to be installed on the condensate line for this unit.
7. The P-traps on the 3rd and 4th floor AHUs need to be modified to allow proper
drainage and also prevent any backflow. The discharge end of the P-trap should
be lower than the supply side.
8. The 3rd and 4th floors have a lxI' hidden spline ceiling panel system. This
significantly reduces access to the ceiling area.
9. All reasonable measures should be taken to control any odors generated during the
cleaning operations.
10. Any dampers which need to be moved during cleaning work shall be marked
initially and then returned to their original position upon completion.
11. Access holes shall be cut into the ductwork as necessary to properly clean all
surfaces. All access holes shall be repaired so as not to affect airflow. Sealing of
holes must be performed in accordance with all applicable codes, and industry
standards using materials acceptable therein.

GENERALSPEC~CATIONS
AIR CONVEYANCE SYSTEM CLEANING
INTRODUCTION
These specifications are necessarily general and are intended only to give a general description of
what is required to adequately complete air conveyance system cleaning projects for the Owner.
Each particular air conveyance system cleaning project is accompanied by a job-specific SCOPE
OF WORK which summarizes the procedures, describes the extent and nature of the material
removal and details any special conditions at the job site. These specifications are not intended to
cover all variations which may occur, however, the SCOPE OF WORK will address anticipated
variations. If any discrepancy arises between these general specifications and the SCOPE OF
WORK, the procedures and instructions in the SCOPE OF WORK will prevail for specific job
requirements .
TERMS AND DEFINITIONS
The following section is a list of terms and definitions that will be used in this specification.
Air Conveyance System (A CS): The air conveyance system is any interior surface of a building's
air distribution system for conditioned spaces and/or occupied zones. The start of the system is
at both the fresh air intake grilles and return air grille. The return air ducts (except ceiling
plenums) to the air handling unit (AHU), the interior surface of the AHU, mixing box, coil
compartment, condensate drain pans, humidifiers, dehumidifiers, supply air ducts, return air
ducts, fans, filters, filter housings, reheat coils, and supply diffusers are all considered part of the
ACS.
Authorized Visitor: Owner's Representative, Consultant, or representative of any regulatory or
other agency having jurisdiction over the project.
Clean Area: An unsoiled area or room with provisions for storage of clean clothing and
equipment.
Coils: Heat-transfer devices that circulate water, brine, refrigerant or steam through tubes in an
air stream to heat, cool, or dehumidify the passing air.
Consultant: Evans Environmental & Geological Science and Management, Inc. who is retained
by Owner to provide consulting services.

Contractor: A contracting company and its employees, which employs a full-time contractor who
is a Florida licensed air conditioning contractor and provides air conveyance system cleaning.
Critical Barrier: A physical barrier which seals openings to the work area in such a way that
airborne particulates can not be released.
Debris: Any particulate substance in the ACS not intended to be present.
Ductwork: A passageway used for conveying air.
Electric Heating Element: A device designed to convert electrical current into heat.
Encapsulant: A liquid material which can be applied to surfaces stripped of foreign matter.
Fan: Power-driven blades that move air continuously by converting rotational mechanical energy
into increased air pressure and air flow.
Fixed Object: A unit of equipment or furniture in the work area which cannot be removed from
the work area.
Foreign Matter: Any organic and/or inorganic substance which is not intended to be present in the
ACS. This includes all debris, dust, or entrained materials.
HEPA Filter: A High Efficiency Particulate Air (HEPA) filter which traps and retains at least
99.97% of monodispersed particles 0.3 microns in diameter or larger.
HEPA-Filtered Unit: A fan which draws air through a HEP A filter and exhausts the filtered air.
HEPA-Filtered Vacuum: High efficiency particulate air filtered vacuuming equipment with a filter
system which collects and retains 99.97 % of monodispersed particles 0.3 microns in diameter or
larger.
Inspection: Verifiable visual quality assurance methods employed to confirm the results of
mechanical cleaning operations.
Mechanical Cleaning: Physical removal of debris and other foreign matter from ACS surfaces.
Moveable Object: A unit of equipment or furniture in the work area that can be removed from the
work area.
Non-porous: A material that does not absorb moisture, such as metal(s) or plastic.

Owner: Monroe County Public Works Division or authorized represemative(s).
Pressure DifferentiaL: A condition whereby the interior of the air conveyance system is maintained
at a pressure differential of at least minus 0.02 inches of water relative to the adjacent exterior
areas.
Pressure Drop: Negative change in static pressure due to friction, dynamic loss or change in
velocity pressure.
Removal: All herein specified procedures necessary to remove foreign matter from the designated
areas within ACS and dispose of these materials.
Static Pressure: The normal force per unit area that would be exerted by a moving fluid on a
small body immersed in it if the body were carried along with the fluid.
Supervisor: An employee of Contractor who is knowledgeable and skilled in techniques and
procedures used to clean, encapsulate, and apply biocides on ACS Projects.
Vacuum Test: A measurable performance test which may be used to verify the cleanliness of any
non-porous surface of an ACS.
VisibLy CLean: Determined by internal visual inspection to be free of foreign matter and other
construction materials.
Wet-Clean: The process of cleaning surfaces and objects by using cloths, mops, or other cleaning
tools which have been dampened with water.
Work Area: The interior air side surfaces of the air conveyance system, specifically air handling
equipment, ductwork, and related in-line components, as well as the floor space beneath or
surrounding the ACS access panels that will be opened or created during the course of the ACS
cleaning project.

SECTION 1.00 GENERAL REQUIREMENTS
PART 1.1 'VORK DESCRIPTION AND CONTRACTOR RESPONSmILITIES
This specification covers cleaning, removal, and disposal of foreign materials from the air
conveyance system located on the 1st, 3rd, and 4th floors of the Monroe County Courthouse
Annex building operated by the Monroe County Public Works Division.
1.1,1 DESCRIPTION OF WORK
A. The work is, in general terms: cleaning, removal, and disposal of all foreign matter
as specified in the SCOPE OF WORK. The foreign matter includes all debris,
dust, or entrained materials in the air conveyance system.
B. This specification covers removal and disposal of foreign matter and all materials
which may have been soiled during disruption of the foreign matter either prior to
or during Contractor's work.
C. The following foreign matter shall be removed and disposed of:
1. All foreign matter accumulated in the air conveyance systems identified in
the SCOPE OF WORK section of this specification.
2. All foreign matter that has accumulated on the exterior surfaces of air
conveyance system equipment, or from decay or disruption of any air
conveyance system equipment.
D. All objects and equipment in the work area must be kept clean and protected by
covering with 1 layer of 6 mil fire-retardant polyethylene sheeting, or removed
prior to disturbance of foreign matter (see SCOPE OF WORK).
E. Related Work
1. Related work includes all work necessary for successful completion of
removal and disposal of foreign matter but not directly involving foreign
matter. This work includes but is not limited to:
a. Protection of the building and property in the building from work
related damage.
b. Proper cleaning and/or disposal of foreign matter and others
materials.

2. Related work includes the maintenance of daily work logs by Contractor on
the job site. These work logs shall be supplied to Contractor by Consultant
and must include:
a. The name of each worker and description of type of respiratory
protection worn by each person.
b. Descriptions of meetings or discussions regarding the job, special
or unusual events, records of daily inspections and records of waste
removal from the work area.
c. A copy of this log, signed by Supervisor must be submitted to
Consultant at the end of the project as a condition for completion.
F. The project shall be termed complete and Contractor released upon satisfaction of
all terms and conditions of this specification, including:
1. All required forms, logs, and receipts and satisfactory completion of
sampling, testing and site inspection by Consultant.
2. A release letter shall be provided to Contractor by Consultant at this time.
3. Satisfaction of all contractual obligations of the Contractor to the Owner.
G. Work Schedule
Upon receipt of notification to proceed with this project by Consultant, Contractor
must obtain all required permits to perform related operations in Monroe County,
Florida. Upon commencement of work, Contractor must complete the project
within the time specified in the SCOPE OF WORK.
1.1.2 CONTRACTOR RESPONSIBILITIES
A. Contractor must furnish all permits, labor, material, services, insurance, tools, and
equipment to complete removal of foreign matter.
B. Contractor must attend a pre-construction meeting to be held at a mutually
agreeable time and date. Attending this meeting will be Owner, Contractor,
Contractor's Supervisor for this project, and Consultant.
1. Contractor's Supervisor who will oversee this project must attend this
meeting.

2. All pre-construction submittals by Contractor will be reviewed at this
meeting. Contractor shall be prepared to discuss and submit plans or
documentation for:
a. Preparation of work area
b. Personal protective equipment
c. Material removal methods and procedures
d. Handling and disposal procedures for foreign matter
e. Final cleanup procedures
f. Sequence and schedule of work
g. Emergency procedures
h. Respiratory Protection Program including evidence of respiratory
protection training and current respirator fit tests.
3. There will be a final walk-through of the building and discussion of plans,
anticipated problems, and areas of special concern.
C. If Owner permits Contractor to use any of its equipment, tools, utilities, or
facilities, such use shall be gratuitous and Contractor shall release Owner from any
responsibility arising from claims or personal injuries, including death, arising out
of the use of such equipment, tools, or facilities irrespective of the condition
thereof or any negligence on the part of Owner in permitting their use.
D. Should Contractor fail or be unable to execute the contract and complete the work
for any reason, then Contractor shall be penalized in accordance with agreements
stated in contract documents.
E. Consultant or Owner retain the right to stop work by and/or dismiss Contractor for
any breach of specified procedures contained in the general specifications.
Dismissal of Contractor may result in claims against Contractor in accordance with
agreements stated in contract documents.
F. Inspections: Inspections of work area will be made by Consultant during the course
of the project. It is the Contractor's responsibility to ensure that:
1. Work area is properly prepared for removal of foreign matter.
2. Foreign matter is being properly removed.
3. Employees of Contractor are properly protected.

4. All foreign matter and other materials have been removed in accordance
with the procedures and SCOPE OF WORK contained in these
specifications.
G. The inspections will in no way eliminate the responsibility of Contractor to correct
any subsequent discoveries of inadequate cleaning, preparation, work procedures,
or remaining material encountered after an inspection, regardless of the outcome
of such an inspection.
H. Supervisory Personnel: Contractor must have a supervisor who is knowledgeable
and skilled in duct cleaning procedures and operations on the job site at all times,
from mobilization to completion of the project.
PART 1.2 SUBMITTALS, NOTICES, RECORD KEEPING, REFERENCES
1.2.1 SUBMITTALS
A. Submit to Owner the following documents with bid proposal:
1. A certificate of insurance issued by Contractor's insurance carrier which
indicates that Contractor holds a commercial/general liability insurance
policy on an occurrence basis for an amount not less than $1,000,000 U.S.
dollars. The insurance carrier must be State of Florida admitted, and rated
at least (A) by a nationally recognized insurance rating agency.
2. A notarized statement signed by an officer of the company, containing the
following information:
a. A list of penalties incurred through non-compliance with similar
project specifications including liquidated damages, overruns in
scheduled time limitations, and resolutions.
b. Situations in which a similar project related contract has been
terminated including projects, dates and reasons for termination.
c. Copies of any notices of intent to initiate enforcement or settlement
agreements such as: Notice of Violation, Notice of Intent to Enforce
Violation, or Consent Agreements.
d. A list of any similar project related legal proceedings/claims in
which Contractor (or employees scheduled to participate in this
project) have participated or are currently involved.

e. A list of other similar companies that current officers of the
company have served as officers in the past five years. Indicate
name, address, and contact at previous companies.
f. A list of the last 5 air conveyance system cleaning projects
completed, their original contract amount, dollar value of change
orders, and name, address, and telephone number of Consultant.
g. A list of term contracts held with institutions or government
agencies, provide list of at least 5 projects completed for the same
owner.
3. A copy of Contractor's drug and alcohol abuse policy.
4. The resumes of the person(s) who are employed by Contractor as
Supervisors that will be used in the air conveyance system cleaning projects
for Owner.
5. A statement that any and all royalties and patents pertammg to air
conveyance system cleaning have been honored by Contractor, or that
Contractor holds Owner harmless from any situation arising from
negligence of Contractor to honor such fees.
6. A copy of the product specifications and material safety data sheets for all
disinfectants, encapsulants, and other chemical agents to be used on this
project for Owner.
B. Upon notification of award of contract, Contractor must submit the following
documents to Owner. Failure to satisfy these submittal requirements will result in
nullification of contract award.
1. A copy of Contractor's Respiratory Protection Program which includes:
a. A statement of Respiratory Protection Training
b. Current copies of qualitative and/or quantitative respirator fit test
results for all employees who will wear respirators on air
conveyance system cleaning projects for Owner. Respirator fit tests
must be performed using respirator brands worn by Contractor's
employees.
c. A copy of the current medical certificates for all employees who
will wear respirators on air conveyance system cleaning projects for
Owner. The certificates must have the name of the medical center,
name of attending physician, and signature of attending physician,

Owner. The certificates must have the name of the medical center,
name of attending physician, and signature of attending physician,
and some indication that the individual is physically capable of
performing work while using a respirator.
2. A copy of Contractor's Worker Protection Procedures.
3. A copy of the emergency procedures and evidence of employee training in
these procedures.
4. A written statement that Contractor will properly dispose of all foreign
matter and other materials generated by this air conveyance system cleaning
project for Monroe County Public Works Division at an appropriate
landfill.
C. Ten working days prior to proceeding with the project, Contractor must submit to
Consultant and Owner the following documents:
1 . A Plan of Action which outlines specific procedures for isolation of work
areas, and performance of cleaning in the work areas.
2. A proposed Schedule of Progress for the specific project.
3. A copy of the report on the "Condition of Building and Fixtures" in work
areas.
D. Upon completion of a specific project, Contractor must submit the following to
Consultant:
1. A waste manifest record of materials disposed of during the project.
2. A copy of all daily logs, sign-in sheets, and daily reports.
3. A list of the names and signatures of all persons involved in the material
removal project.
4. A compliance statement that all materials related to the SCOPE OF WORK
have been removed and disposed of.
E. All submittals to Consultant are expected to be neat, complete, and accurate.

1.2.2 NOTICES TO CONTRACTOR
The following section contains general notices applicable to Contractor for all aIr
conveyance system cleaning work for Owner.
A. Employee Behavior
1. Contractor must provide its employees with a written policy of drug and
alcohol abuse. No employee of Contractor shall be allowed to remain on
Owner's property who is intoxicated by drugs and/or alcohol, or who is
observed using drugs or alcohol on Owner's property.
2. Contractor is expected to enforce its drug and alcohol abuse policy at all
times while conducting business.
3. Weapons, and other hazardous, dangerous, or otherwise disruptive items
in the possession of Contractor or its employees are not allowed on owner's
property.
4. Contractor and its employees are required to display good manners to
owner's personnel and visitors at all times while on owner's property.
Complaints to Consultant or Owner regarding harassment, threatening
behavior, poor personal hygiene, or use of profanity or offensive language
by any employee of Contractor may result in the suspension of project
operations until the behavior problem is corrected.
5. Contractor and its employees are expected to uphold a minimum standard
for street apparel, specifically: Shirts must be worn in areas outside the
work areas, tank tops are not acceptable apparel, and shorts must be at least
knee-length.
B. Performance Standards: Contractor shall perform all material removal operations
using techniques and procedures recognized by the air conveyance system cleaning
industry as being safe and effective in the removal of foreign matter.
C. Pay Requests: All request for payment by Contractor must be submitted to
Consultant on A.LA. forms provided by Consultant.

D. Analytical and Test Results
1. Results of any material sample analyses pertaining to the scope of the
foreign matter removal projects may be available from Owner or Consultant
at Contractor's request.
2. Results of any previous air tests made by Consultant prior to
commencement of work may be available from Consultant upon request
prior to the beginning of the project.
E. Condition of Building and Fixtures: Contractor and Consultant shall agree in
writing on the condition of the building and fixtures, prior to commencement of
work. A report on the "Condition of Building and Fixtures" must be signed and
notarized by both Contractor and Owner prior to commencement of project
operations. Damages incurred by Contractor must be repaired and/or damaged
materials replaced at Contractor's expense.
F. Royalties and Patents: All fees, royalties, and claims for any invention, or
pretended invention, or patent on any article, material, arrangement, appliance or
method that may be used upon or in any manner be connected with the
construction of this work or appurtenances are hereby included in the prices
stipulated in this contract for said work; Contractor hereby expressly binds himself
or itself to indemnify and save harmless Owner from all such claims, fees, and
from any and all suits and actions of every name and description that may be
brought against Owner on account of any such claims, fees, royalties, or costs for
any such invention or patent, and from any and all suits or actions that may be
brought against Owner for the infringement of any and all patents or patent rights
claimed by any person, firm or corporation.
G. Indemnification: Contractor agrees to indemnify, defend, save and hold harmless
Owner from all claims, demands, liabilities, and suits of any nature whatsoever to
the extent they arise out of, or are due to the negligent or wrongful act or omission
by Contractor or its employees.
1.2.3 APPLICABLE REFERENCE DOCUMENTS
The most recent issue of each document is applicable. In case of overlapping jurisdiction
of documents or regulations, the most stringent requirements are applicable.

A. Applicable Regulations
Compliance with all applicable regulations is required. These regulations include,

1.3.1 EQUIPMENT REMOVAL PROCEDURES
Clean external surfaces of containers and equipment thoroughly by wet-cleaning with
sponges, or use HEPA-filtered vacuum before moving such items out of work areas. All
openings on equipment shall be sealed to prevent leakage of foreign matter from
equipment.
1.3.2 SITE SECURITY
Contractor is responsible for all damages to the building or its contents or occupants which
result from the operation of Contractor's equipment or personnel within the building,
except when specified by Consultant or his agent. Damages include, but are not limited
to cleanup of any areas soiled by Contractor during his work and all liquidated damages
as stated and agreed to by Owner and Contractor as a result of Contractor's activities.
1.3.3 SITE CONDITIONS
A. Facilities: Electric power and access to domestic potable water will be made
available for Contractor's use for the duration of the material removal project.
When these utilities are inadequate, Contractor must supply additional utilities.
Arrangement must be made with Owner for use of bathroom facilities during the
project. Bathrooms used by Contractor shall be maintained in clean and sanitary
conditions at all times and be thoroughly cleaned and sanitized before project is
termed complete.

B. Equipment storage: Arrangement for equipment storage will be made during pre-
construction meeting.
C. Dumpster location: Arrangements for location of dumpsters to be made at pre-
construction meeting. All dumpsters must be secured with locks. Dumpsters must
remain locked at all times while present on project property, except when opened
to receive waste, at which times the dumpster is to remain attended by Contractor
personnel.
D. Contractor parking: Arrangements for Contractor employee parking to be made at
the pre-construction meeting.
PART 1.4 PERSONAL PROTECTION
1.4.1 PERSONAL PROTECTION
A. Prior to commencement of work, the workers must be instructed and
knowledgeable on the use and fitting of respirators, on protective clothing, and on
all aspects of work practices and protective measures.
B. Respiratory Protection
1. Provide workers with personally issued and marked respiratory equipment
approved by NIOSH and MSHA.
2. Contractor personnel shall use half-face air purifying respirators equipped
with High Efficiency Particulate Air (HEP A) filter cartridges during
removal of foreign matter, as a minimum.
C. Contractor must provide authorized visitors with similar respirators equipped with
new filters or cartridges whenever authorized visitors are required to enter work
area.
D. Protective Clothing
1. All employees of Contractor, and authorized visitors are required to wear
protective clothing while inside work areas. The protective clothing must
be worn properly. No modifications to the clothing may be made which
exposes the wearer's skin, other than the hands and face.

2. Contractor must provide workers and authorized visitors with sufficient sets
of protective full body clothing. Such clothing will consist of protective full
body coveralls and headgear. Contractor must provide eye protection and
hard hats to all employees and authorized visitors, as required by applicable
safety regulations.
E. Provide and post at job site:
1. The work procedures and personal protection procedures to
be followed by workers.
2. Copies of all OSHA required information, including:
a) Right to Know information; and
b) OSHA standards.
F. Provide and post the location of the nearest hospital, telephone, applicable
emergency phone numbers, and any other emergency information and procedures
for this work.
G. Worker Protection Procedures
1. Each worker and authorized visitor must, prior to entering the work area:
put on protective clothing and respirator.
2. Each worker and authorized visitor must, each time he leaves work area:
a. Remove foreign matter from protective clothing before leaving
work area.
b. Remove all protective clothing and dispose of as waste material.
c. Clean the respirator according to manufacturer's specifications.
3. Workers must not eat, drink, smoke, or chew gum or tobacco in the work
area. Smoking will not be permitted in the building at any time.
H. Protection from Heat Stress: In work areas where heat stress to workers IS
inevitable, Contractor must provide the following:
1. Adequate work breaks in cool areas outside work area, and/or body vests
with ice pack inserts, depending on the site conditions.

SECTION 2.00 MATERIALS AND EQUIPMENT
PART 2.1 MATERIALS
2.1.1 MATERIALS
A. Contractor must furnish all materials and equipment necessary for removal and
disposal of foreign matter in a manner consistent with these specifications.
B. Deliver all materials in the original packages, containers, or bundles bearing the
name of the manufacturer and the brand name.
C. Store all materials subject to damage off the ground, away from wet or damp
surfaces, and under cover sufficient to prevent damage or soiling.
D. Damaged or deteriorated materials shall not be used and must be removed from the
premises.
PART 2.2 TOOLS AND EQUIPMENT
2.2.1 TOOLS AND EQUIPMENT
A. Provide suitable tools for foreign matter removal including, but not limited to,
skipper lines, scrapers, brushes, razor knives, brooms, carts, and safety equipment.
B. Provide equipment and gear, including but not limited to:
1. A method for maintaining a negative differential of 0.02 inches of water
column inside air conveyance system when compared to the outside
2. Portable lighting and power supplies as necessary
3. Indoor portable HEPA-filtered fan units
4. Indoor portable canister-style HEP A-filtered vacuums
5. Mechanical brushes
6. Specialized hand-held duct brushes
7. Personal protection gear
8. Waste disposal bags or containers

SECTION 3.00 EXECUTION
This section applies to the preparation, removal, cleanup, and disposal of foreign matter in air
conveyance systems.
PART 3.1 PREPARATION
This part is intended to be used as a general specification for preparation of work area for any
particular air conveyance system cleaning project for Owner. Consult the SCOPE OF WORK
section for each individual project for more specific preparation requirements.
3.1.1 GENERAL
Proper preparation of the work area prior to air conveyance system cleaning is crucial to
the success of the air conveyance system cleaning projects. The general aspects of
preparation of the work area, as required by Owner are discussed below:
A. Critical Barriers: All air conveyance system cleaning work shall require the
installation of critical barriers at all penetrations to the work area, with the
exception of air-conditioning duct cleaning procedures.
B. HV AC and Electrical Shut Down: HV AC systems serving the work area must be
either shut down or temporarily capped on all air conveyance system cleaning
projects. Electrical systems serving the work area shall be shut down and secured,
or special provisions with Owner must be made to ensure the safety of workers
during project operations. All electrical equipment used by Contractor in the work
area must have GFI circuits.
C. Polyethylene Sheeting: In general, each immediate work area must be protected
from falling foreign matter during material removal, or from damage from
application of encapsulants and/or disinfectants after foreign matter removal.
D. Pressure Differential: The ACS must be placed under a pressure differential of at
least minus 0.02 inches of water column, with respect to the outside of the outside
of the ACS, prior to disturbance of any foreign matter. If an adequate pressure
differential can not be established on small-scale, short-duration work, or in
confined spaces then the intake of HEP A-filtered units must be placed within three
feet of the area where any foreign matter is being disturbed.

PART 3.2 FOREIGN MATTER REMOVAL
This section is intended to be used as a general specification for foreign matter removal in work
area for Owner. Consult the SCOPE OF WORK section for the project for more specific foreign
matter removal requirements.
3.2.1 FOREIGN MA ITER REMOVAL
A. Apply approved biocide inside all appropriate surfaces of the ACS in accordance
with manufacturer's recommendations. Allow at least five minutes of contact time
prior to removal of any foreign matter.
B. Use acceptable procedures to remove foreign matter in small sections and/or areas.
As it is removed, pack the material in sealable, 6 mil polyethylene bags or other
suitable container. Material must not be allowed to be tracked around or dislodged
in work area prior to placement in bags or containers.
C. Waste Load-out Procedure
1. Seal bags or containers. Clean external surfaces of containers thoroughly.
2. Move containers to a holding area.
3. Ensure that containers are removed from holding areas by workers who are
dressed in clean coveralls or clothes.
D. Secondary Removal
1. After completion of gross removal work, all surfaces from which foreign
matter has been removed must be brushed and/or wet-cleaned by an
equivalent method to remove all visible material.
2. Consultant will individually approve each area in wrltIng prior to
commencement of the antimicrobial sealant application. Consultant will
base approval on results of tests and/or visual inspection.
3. Antimicrobial sealant is to be applied only to surfaces from which foreign
matter has been removed and must not be used as a method for sealing dust
on surfaces.

PART 3.3 CLEANUP
This part is intended to be used as a general specification for cleanup of work area for any
particular air conveyance system cleaning project for Owner. Consult the SCOPE OF WORK
section for each individual project for more specific cleanup requirements.
3.3.1 CLEANUP
A. Remove visible accumulations of material and debris. Wet-clean all surfaces within
work area.
B. Remove the fire-retardant polyethylene sheeting, except for HV AC vents which
must remain sealed. Any HEP A-filtered units must remain in place and in service.
C. Clean all surfaces in work area and any other soiled areas with wet-cleaning
methods using amended water, and/or using HEPA-filtered vacuums. After
completion of a second cleaning operation, perform a complete visual inspection
of work area to ensure that work area is free of dust and/or visible material debris.
D. Sealed containers and all equipment in use in work area must be included in the
cleanup and must be removed from work area at an appropriate time in the
cleaning sequence.
PART 3.4 INSPECTIONS AFTER REMOVAL OF FOREIGN MATTER
This part is intended to be used as a general specification for inspections of work area for any
particular air conveyance system cleaning project for Owner. Consult the SCOPE OF WORK
section for each individual project for more specific inspection requirements.
3.4.1 INSPECTIONS AFTER MATERIAL REMOVAL
A. If Consultant finds visible accumulations of foreign matter in work area after the
completion of step 3.3.1 (C), Contractor shall repeat wet-cleaning until work area
is visually clean, at Contractor's expense.
B. When an inspection by Consultant in the presence of Contractor determines that the
area is free of accumulations of dust and visible material debris, the final clearance
criteria has been met, the area thoroughly wet-cleaned, and materials from work
area are removed and disposed of, the area is ready for a final inspection.

C. A final inspection will be carried out by Consultant in the presence of Contractor
to ensure that no dust or debris remains on surfaces as a result of dismantling
operations.
PART 3.5 DISPOSAL
This part is intended to be used as a general specification for disposal of foreign matter for any
particular air conveyance system cleaning project for Owner. Consult the SCOPE OF WORK
section for each individual project for more specific disposal requirements.
3.5.1 DISPOSAL
A. Preparation and Security of Waste Holding Areas
1. Secure transport vehicles and dumpsters with padlocks. Dumpsters and
waste transport vehicles must be locked at all times while engaged in
foreign matter disposal on Owner's property, except when waste materials
are being loaded into these items.
B. Storage and Disposal of Containers
1. Containers of foreign matter removed from the air conveyance system shall
not be stored in occupied areas, but must be moved directly from work area
to an enclosed dumpster or vehicle in enclosed carts.
2. Waste manifests and/or dump receipts must be submitted to Consultant
prior to final clearance of Contractor.
C. Disposal Site
1 . Contractor shall use a disposal site which is operating under all conditions
specified by the Florida Department of Environmental Protection (DEP).
2. Contractor shall notify Owner and Consultant in writing of the location of
the disposal site.

PART 4.1 INSPECTIONS
This section is intended to be used as a general specification for inspections, monitoring, and
completion for any particular air conveyance system cleaning project for Owner. Consult the
SCOPE OF WORK section for each individual project for more specific requirements pertaining
to this section.
4.1.1 INSPECTIONS PRIOR TO AND DURING WORK
A. Contractor shall make all work areas available to inspection by Consultant and/or
Owner at all times, although Consultant agrees not to cause undue delay in the
progress of Contractor's work.
B. Each work area will be inspected by Consultant accompanied by Contractor during
the work.
C. Regular inspections of the HEP A-filtered units will be performed by Consultant to
ensure filters are not overloaded, and are properly seated in HEP A-filtered units.
4.1.2 INSPECTION OF MATERIALS
Consultant may inspect all materials being removed from work area.
4.1.3 FINAL VISUAL INSPECTIONS
A. A final visual inspection will be made after all Contractor's materials have been
removed from work area and all material removal, disposal, and related work is
completed.
B. Contractor shall provide Consultant with a means to inspect all areas of the air
conveyance system (e.g. boroscope, video camera, etc.)
C. All fire-retardant polyethylene sheeting must be removed from work area, with the
exception of critical barriers.

PART 4.2 PROJECT MANAGEMENT
4.2.1 PROJECT MANAGEMENT
A. Owner will employ Consultant to conduct on-site Project Management for all
phases of the air conveyance system cleaning.
B. Consultant will be responsible for:
1. Review of all submittals by Contractor, including pay requests.
2. Conducting inspections at the job site, as required.
3. Performing all final inspections for each project.
4. Submitting final report to Owner which will include all documents, logs,
charts, photographs, and test results pertaining to each project.
PART 4.3 COMPLETION
4.3.1 COMPLETION
A. Completion Criteria
1. After final inspections are complete, Consultant will advise Contractor of
the results.
2. When a work area fails the inspection, the area must be re-cleaned and re-
inspected. The sequence of re-cleaning and re-inspection shall continue
until the area passes the final visual inspection.
B. Re-establishment of Objects and Systems when the project is complete:
1. Relocate all objects moved to temporary locations in the course of the work
to their former positions.
2. Re-establish HV AC, mechanical and electrical systems to proper working
order. Install new filters on HV AC equipment.

SECTION 5.00 ALTERNATE PROCEDURES & VIOLATIONS
This section is intended to be used as a general specification for alternate procedures for any
particular air conveyance system cleaning project for Owner. Consult the SCOPE OF WORK
section for each individual project for more specific requirements pertaining to this section.
PART 5.1 ALTERNATE PROCEDURES
A. Procedures described in this specification must be utilized at all times.
B. When specific procedures cannot be utilized, a request must be made in writing to
Consultant providing details of the problem encountered and recommended
alternatives.
C. Alternative procedures must provide equivalent or greater protection than
procedures that they replace.
D. Any alternative procedure must be approved in writing by Consultant prior to
implementation.
PART 5.2 VIOLATIONS OF SPECIFICATIONS
A. Owner will enforce these specifications through Consultant and Owner's
representative.
B. Owner authorizes Consultant to issue cease work orders upon discovery of any
violation of these specifications. No additional time will be added to the contract
period for time lost due to violations of the specifications.
C. Minor infractions of the specifications may result in cessation of work until the
infraction is corrected.
D. Major violations of this specification may result in the dismissal of the contractor
from all air conveyance system cleaning work, and application of liquidated
damages as stated and agreed to by Contractor in contract documents.

SECTION 6.0 EMERGENCY PLANNING
PART 6.1 EMERGENCY PLANNING
A. Emergency planning must be developed by Contractor and approved by Owner and
Consultant.
B. Emergency procedures must be in written form and prominently posted in plain
view in the work areas. Prior to entering work area everyone must read and sign
these procedures to acknowledge receipt and understanding of work site layout,
location of emergency exits, and emergency procedures.
C. Emergency planning must include:
1. Written notification of police, fire, and emergency personnel of planned
material removal activities, work schedule, and layout of work area.
2. An employee safety meeting must be conducted by Contractor prior to the
commencement of work. The meeting shall be attended by all Contractor
employees on site, and Consultant. All aspects of emergency planning shall
be covered in the meeting.
D. Emergency planning must include:
1. Considerations of fire, explosion, toxic atmospheres, electrical hazards,
slips, falls and trips, confined spaces, and heat related injury.
2. A copy of the emergency procedures and evidence of employee training in
these procedures shall be provided to Consultant.
E. Evacuation and Emergency Procedures
1. Employees must be trained in evacuation procedures in the event of a
workplace emergency.
2. For non-life threatening situations, employees injured or otherwise
incapacitated must remove protective gear, with assistance from fellow
workers if necessary, before exiting the workplace to obtain treatment.

3. For life-threatening injury or illness, removal of protective clothing shall
take least priority after measures to stabilize the injured worker, remove
him from the workplace and secure proper medical treatment.
F. Telephone numbers of all emergency response personnel must be prominently
posted in clean room and equipment room, along with the location of the nearest
telephone and hospital emergency room.
SECTION 7.00 FIRE SAFETY Al\1) SAFE EGRESS
PART 7.1 FIRE PROTECTION-PREVENTION REQUIREMENTS
7.1.1 FIRE PROTECTION PROGRAM
A. Contractor shall be responsible for the development of a fire protection program
to be followed throughout all phases of material removal work, and shall provide
firefighting equipment as specified in this section.
B. As fire hazards occur, there shall be no delay 111 providing the necessary
equipment.
7.1.2 FIRE EXTINGUISHERS
A. Contractor shall provide a fire extinguisher, rated not less than 2A, for each 3,000
square feet of work area.
B. Travel distance from any point of the protected area to the nearest extinguisher
shall not exceed 100 linear feet. This distance shall decrease in areas of limited
mobility.
7.1.3 SPRINKLER SYSTEMS
A. During all air conveyance system cleaning operations, the existing fire sprinkler
system shall be maintained in service at all times.
PART 7.2 SAFE EMERGENCY EGRESS GENERAL REQUIREMENTS
7.2.1 APPLICATION
This part contains general fundamental requirements essential to providing a safe means
of egress from fire and similar emergencies. Nothing in this part shall be construed to

prohibit a better type of construction, more exits, or otherwise safer conditions than the
minimum requirements specified in this part.
7.2.2 FIRE ALARM FACILITIES
A. In each work area, provide fire alarm facilities to workers and other building
occupants for their escape.
B. These fire alarm facilities shall be provided where necessary to warn worker and
building occupants of the existence of fire.
7.2.3 PROTECTION OF WORKERS AND BUILDING OCCUPANTS
A. No existing building shall be occupied during material removal operations unless
all existing exits and any existing fire protection are continuously maintained, or
in lieu thereof, other measures are taken to provide equivalent safety.
B. No flammable or explosive substances or equipment for material removal
operations shall be introduced in a building of normally low or ordinary hazard
classification while building is occupied, provided the condition of use and
safeguards do not create any additional danger or handicap to egress beyond the
normally permissible conditions in the building or work area.
C. Each exit, way of approach, and way of travel from an exit to the street or open
space shall be continuously maintained free of all obstruction or impediments to
instant use in the case of fire or other emergency.
PART 7.3 MEANS OF EGRESS
7.3.1 DEFINITIONS
A. Exit Access: Exit access is that portion of a means of egress which leads to an
entrance to an exit.
B. An Exit: An exit is that portion of a means of egress which is separated from all
other spaces of material removal operations or equipment as a way of travel to the
street or open area.
C. High Hazard Contents: High hazard contents shall be classified as those materials,
substances, or equipment which are able to rapidly burn or from which toxic fumes
or explosions may occur in the event of fire.

7.3.2 EMERGENCY EXITS
A. For each work area, Contractor shall provide an alternate emergency exit.
B. The alternate emergency exit shall consist of a door which leads to a way of exit
access.
7.3.3 EMERGENCY LIGHTING
A. In case of electrical failure during fire, Contractor shall provide battery-operated
lights or lamps in the work area.
B. There shall be at least one battery-operated light or lamp every five workers
present in the work area.

General Insurance Requirements

Other Contractors and Subcontractors
As a pre-requisite of the work governed, or the goods supplied under this contract (including the
pre-staging of personnel and material), the Contractor shall obtain, at hislher own expense,
insurance as specified in any attached schedules, which are made part of this contract. The
Contractor will ensure that the insurance obtained will extend protection to all Subcontractors
engaged by the Contractor. As an alternative, the Contractor may require all Subcontractors to
obtain insurance consistent with the attached schedules.
The Contractor will not be permitted to commence work governed by this contract (including
pre-staging of personnel and material) until satisfactory evidence of the required insurance has
been furnished to the County as specified below. Delays in the commencement of work,
resulting from the failure of the Contractor to provide satisfactory evidence of the required
insurance, shall not extend deadlines specified in this contract and any penalties and failure to
perform assessments shall be imposed as if the work commenced on the specified date and time,
except for the Contractor's failure to provide satisfactory evidence.
The Contractor shall maintain the required insurance throughout the entire term of this contract
and any e>..1ensions specified in the attached schedules. Failure to comply .with this provision
may result in the immediate suspension of all work until the required insurance has been
reinstated or replaced. Delays in the completion of work resulting from the failure of the
Contractor to maintain the required insurance shall not extend deadlines specified in this contract
and any penalties and failure to perform assessments shall be imposed as if the work had not
been suspended, except for the Contractor's failure to maintain the required insurance.
The Contractor shall provide, to the County, as satisfactory evidence of the required insurance,
either:
· Certificate of Insurance
or
· A Certified copy of the actual insurance policy.
The County, at its sole option, has the right to request a certified copy of any or all insurance
policies required by this Contract.
All insurance policies must specify that they are not subject to cancellation, non-renewal,
material change, or reduction in coverage unless a minimum of thirty (30) days prior notification
is given to the County by the insurer.
The acceptance and/or approval of the Contractor's insurance shall not be construed as relieving
the Contractor from any liability or obligation assumed under this contract or imposed by law.

Indemnification and Hold Harmless

Other Contractors and Subcontractors
The Contractor covenants and agrees to indemnify and hold harmless Monroe County Board of
County Commissioners from any and all claims for bodily injury (including death), personal
injury, and property damage (including property owned by Monroe County) and any other losses,
damages, and expenses (including attorney's fees) which arise out of, in connection with, or by
reason of services provided by the Contractor or any of its Subcontractor(s) in any tier,
occasioned by the negligence, errors, or other wrongful act or omission of The Contractor or its
Subcontractors in any tier, their employees, or agents.
In the event the completion of the project (to include the work of others) is delayed or suspended
as a result of the Contractor's failure to purchase or maintain the required insurance, the
Contractor shall indemnify the County from any and all increased expenses resulting from such
delay.
The fIrst ten dollars ($10.00) of remuneration paid to the Contractor is for the indemnification
provided for above.
The extent of liability is in no way limited to, reduced, or lessened by the insurance requirements
contained elsewhere within this agreement.

Prior to the commencement of work governed by this contract, the Contractor shall obtain
Workers' Compensation Insurance v.ith limits sufficient to respond to Florida Statute 440.
In addition, the Contractor shall obtain Employers' Liability Insurance v,ith limits of not less
than:
$100,000 Bodily Injury by Accident
$500,000 Bodily Injury by Disease, policy limits
$100,000 Bodily Injury by Disease, each employee
Coverage shall be maintained throughout the entire term of the contract.
Coverage shall be provided by a company or companies authorized to transact business in the
state of Florida.
If the Contractor has been approved by the Florida's Department of Labor, as an authorized self-
insurer, the County shall recognize and honor the Contractor's status. The Contractor may be
required to submit a Lener of Authorization issued by the Department of Labor and a Certificate
of Insurance, providing details on the Contractor's Excess Insurance Program.
If the Contractor participates in a self-insurance fund, a Certificate of Insurance will be required.
In addition, the Contractor may be required to submit updated financial statements from the fund
upon request from the County.

SECTION IX: SPILL. LEAK. AND DISPOSAL PROCEDURES
ENVIRONMENTAL NOTIFICATION: All spills and leaks
involving more than 10 gallons should be reported
to the nearest regional EPA office or designated
state emergency response office within 24 hours.
spills from ocean vessels or which may
contaminate u.S. coastal waterways should be
reported to the nearest Coast Guard office within
24 hours.
SPILL OR LEAK PROCEDURE: Small spills, involving
less than 10 gallons, may be flushed to a
designated and permitted sewer system with
copious amounts of water. Larger spills should
be contained and neutralized with sodium
bisulfite or sodium thiosulfate (1.2 lbs
neutralizer per each estimated lb. of spilled
material) or disposed of as chemical waste in
manner indicated below. The vicinity of the
spill should be thoroughly flushed with water
after clean-up. At no time should the spilled
material be allowed to dry to a crystalline salt.
Do not discharge this product to storm drains or
to any surface or groundwater source unless
specifically allowed under a valid NPDES permit.
DISPOSAL PROCEDURE: Small quantities, less than
10 gallons, may be flushed to an authorized and
permitted sewer with copious amounts of water.
Larger volumes should be taken to an authorized
chemical disposal site (Class I or landfill) in
accordance with all federal, state, and local
regulations. Consult with selected facility
regarding the need for prior neutralization of
waste.
SECTION X: SPECIAL PRECAUTIONS
PRODUCT STORAGE: Store in a cool, dry, well-
ventilated location away from acids, chlorine and
chlorine compounds, hypochlorites (bleach),
organic solvents, sulfur and sulfite compounds,
phosphorus, combustible/flammable materials, and
direct sunlight. Keep containers tightly closed
when not in use and open carefully to prevent
spillage. Storage on wooden floors and pallets
is not recommended.
PRODUCT HANDLING: Use product only as directed
by the label or by your authorized Bio-Cide
representative. Avoid contact with skin and
eyes; avoid breathing any vapors or fumes
resulting from product activation. Wash
thoroughly after handling. Thoroughly rinse all
protective gear and handling equipment, such as
transfer pumps and lines, with water prior to
reuse or storage. Keep away from children,
animals, and unauthorized personnel.
OTHER PRECAUTIONS: Product may bleach clothing
and fabric materials, such as draperies and
carpets.

INGESTION: Rat Oral LD (50): 4,360 mg/kg.
Ingestion may produce gastric discomfort, nausea,
vomiting, and diarrhea. Intake of large
quantities may produce methemoglobinemia.
EYE CONTACT: Based on rabbit studies, OXINE has
been given an EPA Category III rating as a mild
irritant. Exposure can produce slight irritation
of conjunctiva, cornea, and eyelid.
SKIN CONTACT: Based on rabbit studies, OXINE is
listed as "practically not an irritant".
Prolonged exposure may produce localized
irritation, contact dermatitis, mild erythema,
and edema.
SKIN ABSORPTION: Highly unlikely to be absorbed
through skin in toxic amounts. Rabbit Acute
Dermal LD(SO) > 2,020 mg/kg.
INHALATION: Acute Inhalation: LC (50) > 5.61
mg/l. Prolonged inhalation of fog or mist
containing OXINE may be irritating to nose and
throat.
SYSTEMIC AND OTHER EFFECTS: None known.
CHRONIC EXPOSURE EFFECTS: May cause localized
irritation to areas exposed to product.
MEDICAL CONDITIONS AGGRAVATED BY EXPOSURE: Skin
disorders, such as dermal allergies and
dermatitis. Exposure to chlorine dioxide
produced by activation can aggravate pulmonary
disorders, such as emphysema.
CARCINOGENICITY: Active ingredients are not
listed by ROTECS, OSHA, IARC, or NTP. No
evidence to date implicating product as
carcinogen or tumor promoter.
~GENICITY: Though product active ingredient
is a chemical oxidant, no evidence to date for
mutagenicity from whole animal or in vitro
studies.
REPRODUCTIVE EFFECTS: No known effects to date.
OTHER HEALTH HAZARDS/HEALTH EFFECTS: None known.

SECTION VII: FIRST AID
TARGET ORGANS: Skin, eyes. For chlorine
dioxide produced from activation: respiratory
tract and exposed mucous membranes.
SYMPTOMS OF OVER-EXPOSURE: Skin and eye
irritation. Exposure to chlorine dioxide from
activation can produce coughing, sore throat,
headache, and dizziness.
FIRST AID
SKIN CONTACT: Wash affected area thoroughly
with soap and water. Remove contaminated
clothing and rinse thoroughly with water
before laundering or discard. If irritation
occurs, seek medical attention.
EYE CONTACT: Flush eyes thoroughly with
water, making certain eyelids are held open.
If irritation or burning persist, seek
medical attention.
IHBALATION: Unactivated OXlNE normally has
no respiratory effects. If exposure to
chlorine dioxide produced from activation
occurs, remove victim to fresh air. Contact
a physician if respiratory distress
continues.
INGESTION: DO NOT INDUCE VOMITING. Contact
a physician or Poison control Center
immediately.

precautions only. They are not intended to
replace or supplant the treatment advice of a
physician or other authorized health care
specialist.
SECTION VIII: CONTROL MEASURES/PERSONAL
PROTECTION EOUIPMENT
VENTI~ION: Open air or good room
ventilation is normally adequate for safe use
of this product. Avoid breathing any vapors
or fumes resulting from acid activation.
RESPIRATORY PROTECTION: In accordance with
OSHA regulations (29 CFR 1910.134 and 29 CFR
1910.1000), fogging or spraying applications
may require worker respiratory protection,
such as: (1) NIOSH/MSHA approved air-
purifying respirators, or (2) NIOSH/MSHA
approved canister/cartridge facial
respirators rated for chlorine/acid vapors or
specified for chlorine dioxide.
EYE PROTECTION: Good manufacturing practice
recommends use of chemical safety goggles for
all applications involving chemical handling.
PROTECTIVE CLOTHING: Good manufacturing
practice recommends that, at a minimum,
rubber, neoprene, or other chemically
impervious gloves be worn for all
applications involving chemical handling.
OTHER PROTECTIVE MEASURES: Product should be
stored and applied in close proximity to a
safety shower, chemical eyewash station, or
other fresh water source.

waterways. Use absorbent to assist with the pick-up of material. Scrape up adhesive and
place in container.

VIII. Special Protection Information
A. Ventilation: Use natural cross-ventilation, local (mechanical) pick-up, and/or general area
(mechanical) ventilation to prevent an accumulation of solvent vapors, keeping in mind that
the ventilation pattern must remove the heavier-than-air solvent vapors from the lower
levels of the work spaces. The ventilation should be sufficient to keep the solvent vapor
concentration below the TLV.
B. Respiratory Protection: With adequate ventilation, respiratory equipment should not be
needed. If adequate ventilation is not afforded, wear respiratory equipment approved for
organic vapors.
C. Skin and Eye Protection: During the manufacture and packaging of this product, impervious
gloves, eye protection, and eye wash facilities may be appropriate. During normal end-
product use, cotton or loop-pile gloves and spectacle-type safety glasses are recommended
to prevent contact with this mastic product.
IX. Special Precautions
A. Precautions to be taken in handling and storage: Store in area suitable for flammable
mixtures. Recommended storage temperature is below 90 degrees F.
B. Other Precautions: Vapors are flammable and are heavier-than-air. Prohibit smoking and
eliminate all other sources of ignition, such as regular electrical tools and appliances,
making sure that pilots on gas-fired water heaters are extinguished.
WARNING: This product contains toluene, a chemical known to the state of California to
cause birth defects or other reproductive harm.

VENTLOK INSULATED ACCESS
DOORS
Each VENTLOK Access Door is made from three one-piece stampings: the door frame, the door itself, and the pan. One-
half inch of fibre glass insulation with a density of 1 pound per cubic foot is contained between the door and the pan, which
are welded together. The door is hung on the frame with heavy loose-pin hinges which permit easy removal of door from the
frame if necessary. Each door is finished with a neutral grey high gloss enamel, and packed in individual cartons.
VENTLOK #100 Latches are used as fasteners to give good compression, and heavy gasketing is used between the frame
and the door to reduce air leakage to a minimum. Ga.3keting 1!a" thick is attached to the back of the door frame to insure a
tight seal between the duct and the door frame. For installations where the tightest seal possible is desired between the duct
and the door frame, it is suggested this gasketing be stripped off the door and our #799 Sealant be used between the door
frame and the duct.

This door is identical in every respect to our standard 12" x 16" insulated access door except for the addition of an inspection
port measuring 5" x 8". This inspection window consists of two pieces of 3/'6" Plexiglas, held firmly in place by special
extruded Neoprene gasketing. The air between the two pieces of Plexiglas eliminates most of the fogging that would take

Used primarily for ambient service to retard heat gain and control condensation
drip. Also used for reducing heat flow for hot water plumbing and liquid heating,
dual temperature piping and for air condition and refrigeration systems. It can be
used for temperatures as low as -165 0 F. It is competitive on an installed cost
basis with other types of insulation for its reccommended service. It is U.V.
stabilized and when used outdoors, it does not require special weather-resistant
protective finish. Unslit IMCOA insulation (IMCOSHIELD) comes prelubricated
for easy installation.
Insulation complies with the property requirements of the following specifica-
tions: ASTM C-534, ASTM E-84 (25/50), UL 723 (25/50), NFPA 255 (25/50),
UBC 42-1, CLASS 1, UL-94 HBF, FMVSS-302, CAN-ULC-S102.2-M88 (25/50)

A. Product Name: IMCOLOCK IIMCOSHIELD
B. Description and Ingredient Information: These products are an
expanded, closed cell, polyolefin type compound. They come in
various sizes and in several wall thicknesses; e.g., pipe insulation.
These products do not contain asbestos. These products also do
not contain ammonia, chlorine, any mercury or its compounds,
cyanide or related compounds.

II. Miscellaneous Information
Each of these products is classified as an "article" according to
Title 29 of the Code of Federal Regulations, OSHA Part
1910.1200(c). They are formed to have a specific shape or design
during manufacturing, have end use functions dependent upon
their shape or design, and do not release any hazardous
chemicals under normal conditions of use.
