HaW'~ Deputy Clerk CY At the May 16, 2006, Board of County Commissioner's meeting the Board granted approval and authorized execution of the following: Amendment No. 001 to OAA Contract No. AA-629 between the Alliance for Aging, Inc. (Area Agency on Aging for Miami-Dade and Monroe Counties) and the Monroe County Board of County CommissionerslMonroe County Community Services (In-Home Services and Nutrition Programs). The purpose of the amendment is to change the maximum units of services and reduce the service unit rate under the contract. Total contract amount will remain the same. ./ Amendment No. 001 to the Alzheimer's Disease Initiative (ADI) Contract No. KZ597 between the Alliance for Aging, Inc. and the Monroe County Board of County Commissioners (Monroe County Social Services/In-Home Services Program). The purpose of the amendment is to reduce/de-obligate the ADI Contract by a total of$15,000.00. Amendment No. 001 to the Community Care for the Elderly (CCE) Contract No. KC571 between the Alliance for Aging, Inc. and the Monroe County Board of County Commissioners (Monroe County Social Services/In-Home Services Program). The purpose of the amendment is to reduce/de-obligate the CCE Contract by a total of approximately $80,000.00. Enclosed are four duplicate originals of each of the above-mentioned, executed on behalf of Monroe County, for your handling. Please be sure to return the fully executed "Monroe County Clerk's Office Originals" and the "Monroe County Finance Department's Originals" as soon as possible. Contract Number KZ 597 Page 1 Amendment 001 THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred to as the "Alliance", and Monroe County Board of Commissioners. The purpose of this amendment is reduce/de-obligate your ADI contract by a total of $15,000. This de-obligation is on a one-time basis and does not affect future funding allocations. 1. This amendment shall begin on May 1, 2006 or the date it has been signed by both parties, whichever is earlier. All provisions in the contract and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform with this amendment. All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the contract are hereby amended to conform with this amendment. This amendment and all its attachments are hereby made a part of the contract.
