At the May 16, 2006, Board of County Commissioner's meeting the Board granted
approval to rescind Modification No. 5 to Disaster Relief Funding Agreement No.
01UN-6P-1l-54-15-007, between Momoe County and the Department of Community Affairs,
Division of Emergency Management, and grante approval and authorized execution of a new
revised Modification No.5, extending the length of time to May 31, 2006 and increasing the
budget an additional amount of $24,794.00.
Enclosed are four duplicate originals of the above-mentioned, executed on behalf of
Momoe County, for your handling. Please forward the fully executed "Monroe County Clerk's
Office Orighlal" and the "Monroe County Finance Department's Original" to our office as
soon as possible. Should you have any questions please do not hesitate to contact this office.

MODIFICA nON NUMBER FIVE TO DISASTER RELIEF FUNDING AGREEMENT
This Modification Number Five is made and entered into by and between the Florida
Department of Community Affairs ("the Department") and Monroe County (''the Recipient") to
modifY that agreement between them dated January 18,2001, and numbered DCA Contract No.
01 UN-6P-II-54-15-007(''the Agreement").
WHEREAS, the Agreement expired June 30, 2004;
WHEREAS, the parties desire to extend the Agreement to May 31, 2006, in order for the
Recipient to be reimbursed for work completed in the Scope of Work, as set forth in Attachment
A; and
WHEREAS, the parties desire to modifY the Agreement to increase the funding.
NOW, THEREFORE, the parties agree to reinstate, extend and modifY the Agreement as
follows:
A. Paragraph (3) of the Agreement is hereby deleted in its entirety, and the following
paragraph substituted in its place and stead for all intents and purposes:
This Agreement shall begin January 18,2001 and shall end May 31, 2006, unless
terminated earlier in accordance with the provisions of paragraph (9) of this
Agreement.
B. Attachment A, Budget and Scone of Work, Funding Summary is hereby deleted and
the following Funding Summary substituted in its place and stead for all intents and
purposes:
