4. 1st Amendment to Task Order 2-Review Services 04/08/2022 AMENDMENT ONE TO TASK ORDER TWOFOR ENVIRONMENTAL & HISTORICAL REVIEW SERVICES BETWEEN MONROE COUNTY AND TETRA TECH, INC. FOR THE VOLUNTARY HOME BUYOUT PROGRAM FOR TASKS AND DELIVERABLES ASSOCIATED WITH THE FLORIDA DEPARTMENT OF ECONOMIC OPPORTUNITY SUB-RECIPIENT AGREEMENT I0092 In accordance with the contract for Environmental & Historical Review Services made and entered on the 17th day of November, 2020, by and between Monroe County hereinafter Voluntary Home Buyout Program, and Task Order Two, dated February 17, 2021. All terms and conditions of the referenced Contract for Environmental & Historical Review Services shall apply to this Amendment One to Task Order Two, unless the Task Order modifies an Article of the Contract which will be specifically referenced in this Task Order Twoand the amendment, addition or modification shall be precisely described. 8th April This Amendment One to Task Order Two is effective on the _____ day of ______________ 2022. 1. In accordance with the Environmental & Historical Review Services contract, attachment A, Scope of Work and Pricing, and Task Order Two the Consultant will perform services as described below: a) Perform special studies, additional assessments, or permitting to secure environmental clearance. These may include, but are not limited to biological assessments, wetland delineations, asbestos surveys, lead-based paint assessments, archeology studies, architectural review, Phase I and II ESAs, USACE permits. This Amendment One to Task Order Two provides additional detail related to Attachment A, Scope of Work and Pricing to describe the asbestos survey tasks included in the contract and Task Order Two as shown above and includes the tasks and per structure pricing as shown below. Asbestos Demolition Survey and Reporting An asbestos inspection of the structures will be performed per the Environmental Protection Agency (EPA) and the Occupational Safety and Health Administration (OSHA) regulations. Bulk samples of suspect Sampling Schemes. All bulk samples will be analyzed by an EPA accredited laboratory, for analysis by polarized light microscopy (PLM) in compliance with guidelines established by the EPA (40 CFR Part 763, Subpart F, Appendix A). It is estimated that the collection and analysis of up to 40 samples will be required for each structure. It should be noted unrestricted access and destructive sampling are required to identify to the extent reasonably possible, all suspect materials. Environmental & Historical Review Services Amendment One of Task Order Two For the Monroe County Voluntary Home Buyout Program Page 1of 4 Since the findings of asbestos may have far-reaching implications, Tetra Tech will conduct a thorough assessment of accessible materials and provide realistic and workable recommendations within the framework of OSHA, EPA and State Regulations. By determining the extent and condition of the asbestos and evaluating possible responses (based on thefindings, applicable regulatory requirements and client needs), Tetra Tech can explore appropriate courses of action. Tetra Tech will identify and evaluate any available information provided by the County prior to field inspections. The initial work will include a review of any available data such as building records, plans and construction specifications, as well as any previous survey and renovation information that may reference the presence of ACMs. After reviewing available information, a trained and EPA accredited asbestos inspector will conduct a survey, which generally involves a visual inspection to identify areas of potentially asbestos-containing building materials and assess their physical condition and obtain bulk samples of suspect materials & laboratory analysis. Bulk samples of suspect materials will be analyzed for asbestos content by a laboratory which is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP) following strict chain-of-custody protocol where they will be analyzed according to EPA Methods including Polarized Light Microscopy (PLM) with Dispersion Staining. Sufficient samples will be obtained to ensure the State of Florida minimum sampling requirements. Sampling procedures, which involve detailed record keeping and photo documentation for proper sample material identification and location, are designed to minimize fiber release. Tetra Tech will provide a project-specific report summarizing findings regarding the type, percentage, observed location and estimated quantities of asbestos containing materials present at the sampled locations. The report may recommend temporary fiber control measures and ultimate remediation procedures. If requested by the County, Tetra Tech will assist with the interpretation and completion of required DEP documentation associated with the Asbestos inspections. Post Abatement Inspection and Reporting If directed by the County, a Post Abatement Inspection and Report will be performed. In addition to the services described above, Tetra Tech will perform a visual inspection of the building following the demolition work of the abatement contractor in order to verify that the ACM identified in the asbestos demolition report has been removed. If provided, we can also verify disposal manifest tickets of the ACM following the removal work. Due to the proposed future demolition of the structures, no indoor air quality testing will be performed. In addition, we assume only visual inspection will be required and no additional samples will be collected for Environmental & Historical Review Services Amendment One of Task Order Two For the Monroe County Voluntary Home Buyout Program Page 2of 4 testing. Following visual site inspection, a brief summary report will be prepared describing the findings and confirming that the identified ACM has been removed. If the initial asbestos demolition survey finds no asbestos containing materials, the post abatement survey may not be required. 2.Prior to beginning the work, individual or grouped requests for asbestos-related tasks on specific parcels will be subject to a Notice to Proceed issued via email from the County to Tetra Tech. Tetra Tech understands that each inspection is expected to begin within 5 business days of each issued Notice to Proceed.Tetra Tech will submit a written inspection report, including recommendations within two (2) weeks of the onsite inspection as requested.The County will provide access to the properties for the performance of the studies. 3.This Amendment One to Task Order Two also provides additional detail related to Attachment A, Scope of Work and Pricing to include the asbestos survey tasks and the per structure pricing as shown below. Tetra Tech will obtain approval from the client prior to incurring any costs for point count analysis. c)Changes to the scope of work may impact the cost estimate and warrant a request for an increase. 5.The Total Budget for Task Order Two is unchanged, and will not exceed $139,685.70 unless provided for in accordance with the contract. April 8th, 6.This Amendment One to Task Order Two shall be effective as of _______________, 2022 and shall remain in effect until such services are no longer required or in accordance with section 9.5 of the contract. 7.All other Terms and Conditions of Task Order Two shall remain, in accordance withthe Contract dated November 17, 2020.
